To request benefits from your insurance company, you need to submit a superbill (i.e., statement for insurance reimbursement) to them that describes what services you received and what you paid for them. You will receive a superbill/insurance reimbursement statement on the 5th of each month by email. The monthly superbill is also available in your client portal. This superbill is what you need to provide to United Healthcare so that you may obtain any benefits that you are entitled to.
This link is the form you fill out to mail to United with your superbill. Mail the form and the superbill to the address on the back of your insurance card. There is a box on the form that says: Please check this box if you want United Healthcare to pay benefits directly to the doctor/provider. Do not check the box so they will send the payment directly to YOU. On the United Healthcare form there is a section that requests provider information. This information is provided on the superbill.
If you are the parent of a college student, your student has received their superbill at the email address they provided. Please ask them to forward you a copy or ask them to give Dr. Dunn permission to send you a copy.
